How to Keep Muscle While Losing Weight

Some of the weight lost on a diet or a GLP-1 medication is muscle. Here is what protects it: protein, resistance training, a sensible pace, body composition scans and supervised care in Lutz, FL.

How to keep muscle while losing weight comes down to four things: eating enough protein, lifting weights two or three times a week, losing at a steady rather than reckless pace, and measuring body composition instead of trusting the scale. Some of the weight that leaves during any diet, and during treatment with GLP-1 medications, is lean tissue rather than fat, and how much depends largely on what you do while losing it. At Advanced Optimal Healthcare in Lutz, FL, every weight loss plan is built around protecting the muscle you have and tracking it with an InBody scan.

The Problem Nobody Mentions When the Scale Starts Moving

When a weight loss medication works, the scale moves quickly. The part that gets less attention is what the lost weight is made of. The body does not draw only on fat in a calorie deficit; it also breaks down lean tissue, including skeletal muscle, especially when protein intake is low and the muscles are not being asked to work. On a bathroom scale, a pound of muscle and a pound of fat look identical, so the problem goes unnoticed until strength, energy or body shape give it away.

Why Muscle Matters for Metabolism, Strength and Keeping the Weight Off

Muscle is metabolically active tissue. It burns calories at rest and is a major contributor to insulin sensitivity. Lose a meaningful amount of it and your resting metabolic rate drops further than the weight loss alone would predict, which makes the lower weight harder to hold and regain more likely. Muscle is also what lets you climb stairs, carry groceries, protect your joints and stay independent as you age. The goal of a good weight loss plan is a lower body fat percentage with muscle preserved or increased, not simply a lower number.

How Much Lean Mass Is Typically Lost

It varies, and the research is more reassuring than the headlines. A 2024 review in Diabetes, Obesity and Metabolism (Neeland et al., 2024) found wide variation across GLP-1 trials: in some studies lean mass accounted for 40 to 60 percent of the total weight lost, while in others it was about 15 percent or less. The same review notes that lean mass includes organs, bone and water, not only muscle, and that the muscle changes seen with these medications appear largely proportional to the weight lost. The medication is not uniquely harmful to muscle, but losing a lot of weight without protecting it has a cost, whatever the method. The response is not to avoid treatment; it is to treat with a plan.

What Actually Protects Muscle

Enough protein

The need for protein rises during a calorie deficit. A meta-analysis in Nutrition Reviews (Kim et al., 2016) found that adults who ate higher-protein diets during weight loss, at or above about 1.0 gram per kilogram of body weight per day, retained more lean mass and lost more fat than those who ate less. We typically aim for roughly 1.2 to 1.6 grams per kilogram per day, adjusted for kidney function and appetite. On a GLP-1 medication, appetite falls sharply, so protein has to come first on the plate.

Resistance training

Muscle that is loaded is muscle the body keeps. A meta-analysis in Nutrients (Sardeli et al., 2018) found that resistance training three times a week during calorie restriction prevented the large majority of the lean mass loss seen with dieting alone. Two or three sessions a week of squats, hinges, pushes, pulls and carries, with weights that feel challenging by the last few repetitions, is enough for most people.

A sensible rate of loss

Faster is not better. A loss of about 1 to 2 pounds per week, the pace public health guidance generally recommends, gives the body time to draw on fat stores rather than tissue.

Medical supervision

Supervision ties the pieces together: the right medication and dose for your history, labs, a protein and training plan you can follow, and measurements that show whether it is working. That is what separates the Medical Weight Loss program at Advanced Optimal Healthcare from a prescription alone.

Why the Scale Is the Wrong Instrument

A scale reports total mass. It cannot tell you whether you lost 10 pounds of fat or 6 pounds of fat and 4 pounds of muscle, and it shifts by pounds a day with water alone. An InBody scan uses bioelectrical impedance to report skeletal muscle mass, body fat mass and percentage, basal metabolic rate, body water and a visceral fat estimate, with a segmental breakdown of each arm, each leg and the trunk. Comparing one scan with the next is how we know whether a plan is protecting muscle. Scans are from $45, with packages available, and the scan is included in the $299 initial weight loss consultation.

Where Emsculpt Neo Fits

Diet and lifting protect muscle; Emsculpt Neo builds it. The device combines high-intensity focused electromagnetic energy (HIFEM) with synchronized radiofrequency in a 30-minute session, contracting the targeted muscle at an intensity voluntary exercise cannot reach while the radiofrequency heats the overlying fat (BTL Industries, Emsculpt Neo technology). It is FDA cleared for muscle building and fat reduction in the abdomen, buttocks, flanks, arms, thighs and calves. A typical course is four sessions, 5 to 10 days apart, with no downtime, and results build over one to three months.

For someone losing weight, building muscle in the core and limbs may support the goals that matter in daily life: strength, posture, core stability, recovery and range of motion. Emsculpt Neo does not replace protein, training or supervision, and we do not present it as a treatment for medication-related muscle loss. What it does is build muscle. Individual results vary. Sessions are from $850 and the consultation is complimentary, so if you want to rebuild muscle while the weight comes off, ask about Emsculpt Neo at your consultation.

Where Peptides Fit

Peptide therapy is a provider-supervised option for some patients whose goals include recovery, muscle preservation, sleep and metabolic health. The peptides used are selected after your labs and consultation, never off a menu. At Advanced Optimal Healthcare, peptide therapy begins with a $250 initial consultation that includes labs and an InBody analysis, paired with exercise and nutrition planning. Peptides are not the foundation of muscle preservation; protein, resistance training and a sensible rate of loss are.

What a Supervised 12-Week Program Looks Like

Our medical weight loss program runs 12 weeks and costs $1,999, after a $299 initial consultation that covers your assessment and baseline labs and includes the InBody scan. You then meet with Dr. Ortiz weekly for an hour for meal and exercise planning, nutrition and behavioral counseling, vitamin supplementation and prescription medication when appropriate. For patients who meet the label criteria, a BMI of 30 or above, or 27 or above with a weight-related condition, that medication may be semaglutide, a weekly GLP-1 receptor agonist injection, or tirzepatide, a weekly dual-action GIP and GLP-1 receptor agonist injection. Both are prescribed alongside protein targets, a training plan and repeat InBody scans, because the medication creates the deficit and the plan decides what the deficit is made of. Our obesity treatment page explains how we approach obesity as a chronic medical condition rather than a failure of willpower.

Frequently Asked Questions About Keeping Muscle While Losing Weight

Do GLP-1 medications cause muscle loss?

Some of the weight lost on GLP-1 medications is lean mass, but the amount varies widely between studies and people. A 2024 review found lean mass ranging from about 15 percent to as much as 40 to 60 percent of total weight lost across trials, with muscle changes largely proportional to the weight lost. Individual results vary.

How much protein do I need while losing weight?

More than the standard recommendation for weight maintenance. Research in adults over 50 shows better lean mass retention at or above about 1.0 gram of protein per kilogram of body weight per day, and supervised programs, including ours, often target roughly 1.2 to 1.6 grams per kilogram spread over three or four meals, set individually and adjusted as your weight changes.

What is a good body fat percentage?

There is no single right number; it depends on age, sex and your goals. Use the ranges printed on your InBody report, which are set for your age and sex, and focus on direction: fat percentage down while skeletal muscle holds or rises. Visceral fat, around the organs, is the number most closely tied to metabolic risk.

Is the scale a good way to track progress?

No, not on its own. A scale measures total weight and cannot distinguish fat from muscle or water, so it can show progress while muscle is being lost and a stall while composition is improving. Track body composition with a scan every few weeks and track strength in the gym; those two measures tell you what the weight change is made of.

Can you build muscle while in a calorie deficit?

Yes, particularly if you are new to resistance training, carrying a higher body fat percentage, or returning after time away. It requires adequate protein, progressive resistance training and a moderate rather than extreme deficit. Emsculpt Neo adds a direct stimulus to the treated muscles. Experienced lifters at low body fat find it harder. Individual results vary.

How often should I have a body composition scan?

A baseline scan before you start, then a repeat roughly every four to eight weeks during active weight loss, is a sensible cadence for most people, because meaningful changes in muscle and fat take weeks to show. In our 12-week program the scan is built into the initial consultation and repeated to track progress.
