Advanced Optimal Healthcare is a membership-based direct primary care practice in Lutz, FL, founded in 2020 and led by Dr. Anilda Ortiz, DNP, APRN, FNP-C, a board-certified family nurse practitioner. Instead of billing insurance for office visits, the practice offers a flat monthly membership that gives adults and families longer, unhurried appointments and direct access to their provider. A separate wellness track covers services such as hormone health and medical weight loss for people who want more than routine care.

A Different Kind of Primary Care in Lutz, Florida

At Advanced Optimal Healthcare, we believe good medicine starts with listening. Whether you are managing a long-term condition or simply looking for a provider who takes your concerns seriously, the difference shows up at the first visit: time to tell your whole story, questions answered in plain language, and a plan built around your life rather than a template.

The practice opened its doors on October 1, 2020, to help address the primary care shortage in our area and to offer personalized, affordable care close to home. It is Latina- and woman-owned, and our small team cares for adults and families from Lutz, Carrollwood, Northdale, Cheval and the surrounding Tampa area. Our mission is to provide a warm, comfortable, judgment-free environment where we take care of our members the way we would care for our own family. You can read more about our story and values on the About Advanced Optimal Healthcare page.

What Is Direct Primary Care?

Direct primary care (DPC) is a model in which you pay your primary care practice a flat monthly fee instead of paying per visit through insurance. Because the practice does not bill insurance for office visits at any membership level, there are no copays, no deductibles and no surprise bills for in-office care. That frees the schedule from the volume pressure that drives rushed visits, so your provider can spend real time with you.

Direct primary care is not a replacement for health insurance. Members keep their insurance for the things a primary care office does not provide, such as specialist visits, hospital care, urgent care, imaging and many prescriptions. Through membership, you also gain access to discounted pricing on labs, imaging and medications, and blood draws are done right in our office. If you are weighing different membership-style practices, our article on how direct primary care compares with other membership models walks through the differences.

Two Membership Tracks: Primary Care and Wellness

Advanced Optimal Healthcare offers two membership tracks, and they are separate. You choose the one, or both, that fits your goals.

Primary Care Memberships

  • Individual: Foundational $99, Advanced $199 or Optimal $299 per month
  • Family (Foundational level): $159 per month for two people, $320 per month for four, and $80 for each additional person. An individual inside a family plan may hold the Optimal level at the Optimal price.

Every primary care membership includes unlimited routine and sick visits, direct access to your provider by phone, secure text, video chat and email, longer appointments, telehealth visits, preventive exams, up-front transparent pricing and discounted labs, imaging and medications. Members also receive priority scheduling and discounted pricing on wellness services.

Wellness Memberships

  • Foundational $399, Advanced $499 or Optimal $699 per month

Wellness memberships are a distinct track for people focused on goals such as body composition, energy and overall wellness. What each tier includes is reviewed in person, so the best way to compare levels is the 20-minute complimentary consultation. Memberships are billed monthly, can be canceled with 30 days' notice and may be HSA eligible.

Meet Dr. Anilda Ortiz, DNP, APRN, FNP-C

Dr. Ortiz, our founder and family nurse practitioner, is originally from Providence, Rhode Island. She has lived in Florida since 2002 and has been part of the Tampa Bay medical community since 2005. She earned her Bachelor of Science in Nursing at the University of Rhode Island, then her Master of Science in Nursing (Family Nurse Practitioner) and her Doctor of Nursing Practice at the University of South Florida in Tampa. She is board certified as a family nurse practitioner and licensed as an Advanced Practice Registered Nurse in Florida and Rhode Island.

Her clinical experience includes:

  • Primary care, internal medicine and family practice for more than 14 years
  • Endocrinology, diabetes and thyroid care for more than 10 years
  • Wellness and preventive care for more than 5 years

Before becoming a nurse practitioner in 2011, Dr. Ortiz spent eight years as a pediatric registered nurse. If you are new to the role, the American Association of Nurse Practitioners explains what a nurse practitioner is and the care NPs provide. Dr. Ortiz lives in the Lutz and Land O' Lakes area with her husband and their two children, and she brings both deep clinical knowledge and a genuine sense of connection to every patient she sees.

What Makes Our Approach Different?

No rushed visits and no one-size-fits-all plans. Just focused time with a provider who builds a care plan around your life, your concerns and your goals. A few things patients notice right away:

  • One patient at a time. We schedule one patient per time slot, so you are seen on time and the visit is not cut short.
  • Lab literacy. Dr. Ortiz walks through your results line by line, so you understand what each number means and why it matters for your plan.
  • Direct access. Members reach their provider by phone, secure text, video chat and email, and can get seen when they need to, in the office or by telehealth.
  • Prevention first. The focus is on catching problems early and keeping you well, not only on treating illness once it appears.
  • Partnership. You leave each visit knowing the plan, the reasons behind it and the next step.

Services We Offer, From Annual Physicals to Hormone Health

Primary care is the foundation. Members come to us for annual physicals and preventive care, sick visits and ongoing chronic care and medication management for conditions such as diabetes, thyroid disease, high blood pressure and high cholesterol. When you need a specialist, imaging or hospital care, we coordinate with the rest of your care team.

Alongside primary care, our wellness services in Lutz are designed for people who want to feel and function better. They include a supervised 12-week medical weight loss program with lab work, an InBody scan for body composition and weekly sessions with Dr. Ortiz, as well as IV therapy, vitamin injections and body contouring options. For hormone concerns such as fatigue, low libido, hot flashes or changes in mood, lab-guided hormone therapy for men and women starts with testing and a full symptom review before any treatment is discussed. Results with any wellness or hormone service depend on the person. Individual results vary.

What Your First Visit Looks Like

Most people start with the complimentary consultation, a 20-minute meet and greet to review our services, answer your questions and see whether the practice is a good fit. There is no exam at this conversation and no pressure to join.

If you decide to become a member, your first full visit is longer than what many people are used to. Dr. Ortiz reviews your health history, current medications and family history, talks through your goals and examines you. Blood work can be drawn in the office, and certain lab tests can be run on site. Once results are back, she reviews them with you and outlines a plan that may include preventive screenings, medication changes, lifestyle steps or a referral. Our article on what to expect from your first visit covers the process in more detail.
